Subacute Care
Subacute care is a comprehensive, cost-effective, inpatient level of care for patients who have had an injury or acute (sudden onset) illness, a disease episode, or who may need and could benefit from restorative care. Resident treatment may require diagnostics, rehabilitation, or invasive procedures. The treatment has an expected outcome of achieving a high level of independence. Often, subacute care immediately follows hospital medical care and can serve as a bridge between hospital and home.
The Subacute Wing at Glacier Hills
A newly renovated wing of the Nursing Center serves to provide specialized subacute care from a team of medical professionals, which includes Rehabilitation Therapists, a Nurse Case Manager, Geriatric Physicians, Nurse Practitioners, specially trained Social Workers, and a Registered Dietitian. The subacute area includes a solarium and was designed with bright and cheerful common areas where patients and guests can relax and visit. The subacute wing is close to the therapy suites to maximize rehabilitative time.
